THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN OBTAIN ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
The terms of this Notice of Privacy Practices (“Notice”) apply to LABYRINTH PLLC, its affiliates, and its employees. While uncommon, LABYRINTH PLLC may share protected health information of clients as necessary to carry out treatment, payment, and health care operations as permitted by law.
We are required by law to maintain the privacy of your protected health information and to provide notice of our legal duties and privacy practices. We will abide by the terms of this Notice for as long as it remains in effect.
We reserve the right to change this Notice and make the revised notice effective for all protected health information we maintain. We are required to notify you in the event of a breach of your unsecured protected health information. Some state privacy laws may be more stringent than HIPAA requirements.
Copies of revised notices or state-specific information may be requested from the Privacy Officer.
Uses and Disclosures of Protected Health Information
Authorization and Consent
We will not use or disclose your protected health information without your authorization except for treatment, payment, and health care operations, or as otherwise permitted or required by law. You may revoke authorization in writing, except where action has already been taken or where insurance laws apply.
Treatment
Information may be shared among healthcare professionals involved in your care to provide, coordinate, or manage treatment.
Payment
Information may be used or disclosed to obtain payment for services, including billing and insurance processing.
Appointments and Communications
We may contact you regarding appointments, treatment information, or health-related services. Communications may occur via email, phone, or SMS/text. By receiving services, you consent to these methods.

Research
Protected health information may be used for research in limited circumstances with appropriate safeguards.
Professional Consultation
Your therapist may consult with a licensed clinical supervisor to ensure best practices.
Professional Will
In the event of therapist incapacity, directions may exist to ensure continuity of care or referrals.

To report abuse, neglect, or domestic violence
To the FDA for safety reporting

Psychotherapy Notes
Written authorization is required to disclose psychotherapy notes except in limited circumstances permitted by law.
Marketing
Authorization is required for marketing disclosures, except for face-to-face communications or nominal gifts.
